The Lead Case Manager provides monitoring services and treatment adherence support. Responsible for court system coordination. Plans for admission, treatment, discharge and aftercare. Provides screening services as needed. Maintains contact with referring agencies and related people and with community resources used for referral. Maintains coordination with Screening, local and state hospitals, treatment providers, and other treatment team members for consumers. Maintains positive working relationships with community agencies. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. Individual must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide care based on agency approved evidence based practice models.
