Reduced full-time APN will conduct initial and follow up psychiatric consultations to the patients of Shore Medical Center. APN will function as a part of an interdisciplinary team with hospitalists, specialists, nurses, social work and other health care staff. APN is expected to have concurrent documentation. Kairos House is a short-term residential program located in Burlington County that offers a safe, therapeutic option for those who are at risk of hospitalization due to exacerbation of their mental illness; or for those who need further stabilization and/or a transition period following an inpatient stay. Services include case management, treatment, housing assistance, wellness groups and life skills training. The Support Specialist provides mental health services to adults who have a chronic mental illness in a respite setting. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. Provides direct supervision of the consumers residing in the residential setting. Provides crisis intervention as needed. Collaborates with community resources to link consumers to appropriate mental health and substance abuse providers. Provides social skills groups related to the consumer’s individual needs.
