Conducts psychiatric evaluation/medical management and related therapeutic services. Aids in formulating diagnostic impressions based on DSM-V criteria in collaboration with the psychiatrist and other treatment members. The Case Manager provides age related case management to all consumers in the assigned program. Maintains and helps establish contact with referring agencies, other providers, and family-team members. Coordinates educational services for all residents of the program, and acts as educational liaison to the schools, handling educational and vocational needs. Is responsible for overseeing intake documentation is completed, and that all required documentation is collected. Directly coordinates the medical care for the consumers in the program, including verifying insurance information, obtaining late authorizations, identifying medical providers, and scheduling all necessary medical appointments; ensuring regulatory compliance on these items. Records all case management activity in the consumer’s medical record, and communicates it appropriately to all staff in the home.
