Aledade The clinical Pharmacist of Patient & Practice Solutions will be responsible for leading the transformation of practices "upstream" by implementing a hybrid model of care blending clinical intervention with operational consulting. Supporting specific regional market field teams, practices, and patients, the pharmacist drives performance for medication adherence and other pharmacy-related measures. By engaging directly with patients, the pharmacist serves as a seamless extension of the practice care team, fostering trust and continuity.
