Job Summary: Evaluates programs and services to meet the needs of patients with cancer. Case manages selected populations of patients diagnosed with cancer using variety of skills, psychosocial and education interventions. Functions as liaison among all departments and services providing care for patient. Educates members, patients, staff and community about cancer. Essential Responsibilities: Develop programs to improve the overall journey of the cancer patient; providing timely and coordinated care with a multidisciplinary approach. Areas of accountability include but are not limited to; patient navigation, peer navigation, timely access to care, psychosocial support, access to community resources, patient education and pain management.
