The duties of the Community Support Specialist include contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. The Therapist conducts trauma informed integrated health outreach, screenings, assessment/evaluations, orders, care management, linkages for individuals eligible for rural preventive health initiatives including mobile clinic services. Works with multidisciplinary team and community partners to outreach and engage individuals into care and treatment.
