Conducts initial Social Determinants of Health screenings for individuals in jail and community settings and provides orientation to Diversion Support Services. Delivers peer support services grounded in lived experience using evidence based practices, including Mary Copeland’s Wellness Recovery Action Plan (WRAP) and principles consistent with Forensic Assertive Community Treatment (FACT). Provides services in office based and community based settings, including field based outreach and engagement. Utilizes agency vehicles to transport clients, when clinically indicated, to identified needs such as treatment appointments, housing resources, benefits offices, and other recovery related services. Responsible for peer community support service provision including working in collaboration with the service team in the development, implementation, monitoring, and updating of the court ordered plan; therapeutic rehabilitative skill development; and, illness management and recovery. Assists consumers with community integration, goal setting, and wellness planning. Delivers age and culturally appropriate services to program consumers. Provides supportive peer counseling to adults prior to hospital discharge and within the IOC Program. Demonstrates knowledge of mental illness, substance abuse and the services available to serve those populations within the assigned County. Candidate must have ability as survivor of mental illness to provide to other consumers their unique experiences and serve as a role model.
