The Lead Case Manager provides monitoring services and treatment adherence support. Responsible for court system coordination. Plans for admission, treatment, discharge and aftercare. The Crisis Specialist interviews consumers, completes assessment, treatment plans, and crisis intervention. Outreaches as required. Answers a 24 hour hotline. Expected to report for work in inclement weather. Additional tasks will be added at Director’s discretion. Legacy operates the State-designated Screening and Crisis Intervention Program for Burlington County, providing evaluations 24 hours a day, 365 days a year to individuals experiencing a psychiatric crisis or posing a danger to self, others or property. We make it our priority to screen and triage individuals in crisis within community settings as opposed to a hospital setting. Our certified screeners and mental health professionals work together to stabilize the crisis and determine the most appropriate, least restrictive treatment option.
