Responsible for peer community support service provision including working in collaboration with the service team in the development, implementation, monitoring, and updating of the court ordered plan; therapeutic rehabilitative skill development; and, illness management and recovery. Assists consumers with community integration, goal setting, and wellness planning. Delivers age and culturally appropriate services to program consumers. Provides supportive peer counseling to adults prior to hospital discharge and within the IOC Program. Demonstrates knowledge of mental illness, substance abuse and the services available to serve those populations within the assigned County. Job Summary: The Peer Support Specialist for the Diversion Support Services (DSS) program provides recovery-oriented support to individuals involved in the justice system. This role includes completing Social Determinants of Health (SDOH) screenings within the jail setting and offering an introduction to the DSS program. Drawing from lived experience with mental health and/or substance use challenges, the Peer Specialist engages individuals through individual and group sessions to foster hope, empowerment, and recovery. The Peer Specialist uses the Wellness and Recovery Action Plan (WRAP) model and other evidence-informed tools to support participants in building coping skills, navigating reentry, and connecting to services.
