The duties of the Community Support Specialist include contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Improves client well-being and functioning by assisting clinical case manager in the coordination of services that support TRAIL clients.
