The APN conducts trauma informed integrated health outreach, screenings, assessment/evaluations, orders, care management, linkages and medication evaluations for individuals eligible for rural preventive health initiatives, including mobile clinic services. Works with multidisciplinary team, community pharmacy and partners to outreach and engage individuals into care and treatment. Provide preventive chronic disease screenings, brief intervention/health education services and referrals/linkages to primary care, behavioral health including substance use disorder treatment. Aids in formulating diagnostic impressions based on DSM-V criteria in collaboration with the psychiatrist, therapist and other treatment members. Develops care plans for individuals requiring ongoing care management including health coaching, specialty care, medication screening and adherence support.
