The Peer Recovery Specialist completes the initial Social Determinants of Health (SDOH) screening for individuals in a community setting and provides introduction to program. Provides support to individuals from a mental health lived experience perspective in both individual and group formats. Able to utilize the Wellness and Recovery Action Plan model. This role includes completing Social Determinants of Health (SDOH) screenings within the community setting. Drawing from lived experience with mental health and/or substance use challenges, the Peer Specialist engages individuals through individual and group sessions to foster hope, empowerment, and recovery.
