The In Community Therapist will assess need for treatment, document and deliver clinical evidenced- based behavioral health services to youth and their families in a community-based setting. Design and provide clinical interventions, teach staff and actively participate on the treatment team to enable youth to reach their highest levels of independence in the community. Will perform all duties necessary to meet state approved objectives, including the requirement that the employee cooperate with the licensee and Department of Children and Families (DCF) staff in any inspection or investigation. The Life Skills Coach improves client well-being and functioning by assisting clinical case manager in the coordination of services that support TRAIL clients. Support services include helping clients 1) master an array of needed life skills, 2) to actively participate in community activities, 3) to provide alternative settings and means to explore clinical issues using direct care interventions based on treatment goals and objectives, and 4) conducting assessments including the Casey Life Skills Assessment.
