The duties of the Community Support Specialist include contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements; other duties as assigned by management staff. Expected to report for work in inclement weather. Additional tasks will be added at Director’s discretion. Legacy operates the State-designated Screening and Crisis Intervention Program for Burlington County, providing evaluations 24 hours a day, 365 days a year to individuals experiencing a psychiatric crisis or posing a danger to self, others or property. We make it our priority to screen and triage individuals in crisis within community settings as opposed to a hospital setting. Our certified screeners and mental health professionals work together to stabilize the crisis and determine the most appropriate, least restrictive treatment option. Services include case management, treatment, housing assistance, wellness groups and life skills training. Improves client well-being and functioning by assisting clinical case manager in the coordination of services that support TRAIL clients.
