The Lead Case Manager provides monitoring services and treatment adherence support. Responsible for court system coordination. Plans for admission, treatment, discharge and aftercare. Provides screening services as needed. Maintains contact with referring agencies and related people and with community resources used for referral. Delivers peer support services grounded in lived experience using evidence based practices, including Mary Copeland’s Wellness Recovery Action Plan (WRAP) and principles consistent with Forensic Assertive Community Treatment (FACT). Provides services in office based and community based settings, including field based outreach and engagement. Utilizes agency vehicles to transport clients, when clinically indicated, to identified needs such as treatment appointments, housing resources, benefits offices, and other recovery related services.
