Admissions Coordinator

Job Description

Position will maintain the census of Legacy Treatment Services Residential Treatment Home Programs at optimum levels. Additionally, the Admissions Coordinator will coordinate and schedule meet-and-greets, admission interviews, and intake assessments in collaboration with internal/external treatment teams, program leadership, Case Managers, and families. Admissions Coordinator will also ensure smooth communication among internal departments to support timely and effective admissions.

Additional responsibilities include but are not limited to:
  • Collaborates with Perform Care and all the various State agencies who refer Consumers to the program.
  • Will ensure that the Consumers being referred are appropriate for our therapeutic program by reviewing evaluations and conducting preplacement conferences.
  • Responsible for representing the Agency when meeting with parents, Case Managers and potential residents to provide an overview of the Agency.
  • Review Youth Link Bulletin Board on a daily basis to identify and develop potential referrals.
  • Coordinate and communicate with the Admissions Team regarding all appropriate potential admissions.
  • Responsible for reporting admissions and obtaining payment authorization from Perform Care in a timely manner.
  • Prepare weekly Perform Care Authorization Report for Clinical Operations Meeting.
  • Prepare weekly Children's System of Care reports for Program Leads within Office of Residential Services (ORS).
  • Update weekly admissions activity report.
  • Completes monthly admissions statistical report in a timely manner.
  • Must be knowledgeable regarding State, Medicaid, and Department of Youth and Family Services (DYFS), especially pertaining to admission requirements.
  • Must be knowledgeable regarding all aspects of Agency admissions, policy and procedures, as well as Agency wide policies and procedures in order to be able to convey information to Case Managers, families, potential residents, or any interested parties.
  • Actively participate in ongoing quality improvement activities of Legacy Treatment Services including, but not limited to, serving as a member of the Policy and Procedure Committee, peer record review and program evaluation/quality improvement projects, etc.
  • Responsible for visiting other potentially referring agencies and informing them about Legacy Treatment Services.
  • Attend all appropriate meetings both within the Agency and State level.
  • Maintain and revise admissions database as needed to provide the most accurate and useful information pertaining to admissions.
  • Responsible for completing the admissions assessment, point of entry, bed search and primary person within the electronic health record.
  • Responsible for training others in the Admissions electronic health record process.
  • Facilitate the transition of residents into the appropriate educational setting, coordinate with internal education staff and external school districts to ensure timely school placement and continuity of educational services.
  • Conduct outreach visits to external agencies and providers to promote Legacy Treatment Services’ programs and strengthen referral relationships.
  • Support other staff assigned to Admissions operations, provide guidance, feedback, and training as needed
  • Assist with training Crisis Stabilization and Assessment Programs Transition Specialists on admissions workflows and electronic health record processes related to admissions documentation
  • Train staff on admissions workflows and electronic health record processes related to admissions documentation.
  • Ensure adherence to Agency policies, confidentiality standards, and applicable regulations during all admissions related processes.
  • Perform additional administrative and operational duties as assigned.
Hours:

Full time; Salary is commensurate with experience and education.

Education, Training & Experience:

High School Diploma required. Associate or Bachelor’s Degree in social services, psychology, or related field preferred. Experience in admissions, case coordination, or human services preferred, but not required.
