Lead Support Specialist

Job Description

Provides services to adults with serious mental illness in an acute care residential setting, serving as a Lead Support Specialist responsible for mentoring and training incoming staff while ensuring high-quality consumer care. Delivers comprehensive case management services, including participation in treatment planning, discharge preparation, and service linkage for individuals transitioning to community settings. Supports with conducting residential intake assessments and collaborates with interdisciplinary team to update and implement individualized treatment plans. Manages external appointments, court obligations, medical coordination, and benefits navigation for complex care consumers, including providing transportation to ensure continuity of care. Engages in continuous, ongoing training and professional development to maintain competency and enhance quality of care. Assesses and interprets consumer data to identify needs and deliver appropriate, individualized, and age-related care. Collaborates effectively with interdisciplinary teams and internal/external stakeholders to support holistic care. Participates in and supports continuous staff training, onboarding, and skill development initiatives. Performs additional duties and responsibilities as assigned by management.

BA/BS Degree must be in mental health discipline or health/human services or one (1) year, full time experience working with people diagnoses with serious
mental illness. AA/AS with two (2) years' experience or High School Diploma with four (4) years' experience. Employee must demonstrate the personal characteristics and experience to collaborate with and provide appropriate care to Consumers, gain their respect, guide their development, and participate in
their overall treatment program; the ability to support constructive Consumer-family visitation and Consumer involvement in community activities; the temperament to work with, and care for, adults with special needs, as appropriate; and the ability to work effectively with the treatment team and other
internal and external stakeholders. Related work or life experience is defined as “activity in a supportive role with an individual providing assistance, advice, guidance, instruction, or direction in completing activities of daily living or coping with ongoing stressful life circumstances.”

Excellent communication and interpersonal skills required. Employee must possess a valid Driver’s License and, if duties require driving, be able to drive an Agency owned vehicle. Attend monthly mandatory staff meetings. In the event of inclement weather and/or State of Emergency, all employees are expected to report to work as scheduled. Part-time employees are required to work a minimum of 2-3 shifts per month and minimum of 2 holidays per year. Must be willing to work weekends and overnights.
