Community Health and Counseling Services is a non-profit organization that was established in 1883. We offer behavioral health, crisis, home health and hospice services for children & adults through Maine communities.

One of the few combined home health and behavioral health care agencies in the United States, Community Health and Counseling Services combines mutually supportive programs and collaborative relationships with other health care providers to serve both the physical and psychological aspects of an individual’s well-being.

CHCS is looking for a full-time person to help develop new foster care providers and develops and monitors support services for existing foster care providers.  Evaluates the performance of contracted foster care providers consistent with the foster care program's standards of care and will participate in the foster care program development.   Must have a minimum of a Bachelor’s Degree from an accredited college or university in psychology, social work, child development, special education, rehabilitation, sociology, human services, or related field.  This position requires a minimum of one year of professional experience working with children and their families who have emotional or behavioral challenges.  Past experience in or knowledge of the foster care system is a plus.    Flexible hours, including evenings and weekends also required along with reliable transportation and valid Maine driver’s license. This position is the core point of contact for foster family engagement in the Resource Parent Care Team. The Resource Parent Care Team Liaison will each work in a designated Office of Child and Family Services (OCFS) District office Monday through Friday from 8:00am to 5:00pm . This person will initiate and build relationships with foster families and other members of the team, including providers, OCFS staff, and informal/formal supports. These individuals are eligible under the Maine Projects for Assistance in Transition from Homelessness (PATH) Program. The PATH Navigator position will be responsible for the identification of eligible individuals, assessment of needs, planning, linking to resources, monitoring and evaluation of services and supports needed by homeless individuals. Associate’s Degree in a human service field with three years relevant experience or a Bachelor’s Degree in a human service field with one year of relevant experience. Must minimally meet MHRT-C provisional certification requirements at time of hire and complete the MHRTC certification requirements within two years of employmen. Must have 2 years experience engaging homeless persons with disabilities. Must have the ability to participate in street outreach, engage people who are homeless, perhaps living outdoors, at shelters, or other community locations.
