Case Manager/QMRP

Responsibilities include: Client care, implementation of Individual Support Plans, documentation, file maintenance & other duties as assigned. Will also serve as a liaison among families, staff, residents & the community. Applicants must have a Bachelor’s degree in a Human Services related field & a minimum of one-year experience working with individuals with developmental disabilities.


 

Group Home Manager

Responsibilities include: Supervise, train & schedule direct care staff, participate in the development & monitoring of client Individual Support Plans; plan & implement community based, age appropriate recreational activities, monitor group home budgets & other duties as assigned. Applicants should have 1+ years experience working with developmentally disabled individuals & have supervisory experience. College degree or related experience is preferred.


 

The Group Home Case Manager & Group Home manager must be willing to work together as a team.
