Coordinated Family Care provides child centered and strength-based care management services while partnering with families to realize their vision, goals, and full potential. As a Care Management Organization (CMO), we partner with families to design, implement, and manage individual service plans (ISPs) using both informal and community support for children with complex needs including behavioral, emotional, and developmental disabilities. The wraparound model focuses on normalization and shifts the balance of decision making from the traditional realm of professionals to the family and community. It strives to decrease families' reliance on state-run agencies and enhance the collaboration between service agency workers, the birth family, extended family, and the community to devise highly individualized plans.
