Develop and implement patient care plans. Provide patient care within an integrated Care Delivery (IGP) model including coordinated care with physicians, family and other team members with regards to diagnosis, intervention and progress. Fulfill all documentation and coding requirements. Follow Kaiser Foundation Health Plan of Washington and departmental policies and procedures. Comply with all regulatory requirements.
