Aledade The Manager, Care Management determines care management strategy and leads the care management team in pursuit of market goals. They ensure compliance with program requirements by liaising with contract and program-specific teams and Aledade. They coach and support partner practices in understanding and succeeding within these programs, provide thought leadership on care management program roles and metrics, manage care team members, and develop and deploy training programs.
