Merakey is currently seeking a Community RN DDTT PRN to join our Merakey Behavioral Health division. McMurray, PA location is the home base for this position. You can choose to work in any area that works best for you. There is a 4 county catchment area for this position (Washington, Beaver, Butler and Lawrence)
SHIFT: PRN (as needed) M-F, 8:30-6:30p as needed
The Dual Diagnosis DDTT Nurse acts as a team player and shares in the responsibility of carrying out rehabilitation and support functions and assisting in treatment, substance abuse services, education, support and consultation to families and crisis intervention services under the clinical supervision of staff with professional degrees. The nurse is specifically responsible for medical monitoring and education for those on the DDTT team. Work collaboratively with Psychiatrist, Program Director, Team members and Treatment Team/IA. Responsibilities include but are not limited to the following.
Assist in the coordination and monitoring of all medications (both medical and psychiatric)
Coordinates all medical care with PCPs, PH MCO, dental and vision providers
Performs periodic health assessments to monitor physical health wellness, medication issues/needs and current fitness and nutritional/dietary needs
Works to develop goals related to physical health wellness, medication management, fitness and nutrition with individuals served and their supports
Participates in on-call schedule on a rotating basis
Provide recovery based, person centered services
Develop comprehensive goal plans in conjunction with individuals and family members.
Provide Service coordination, including coordinating and monitoring the activities of the individual's treatment team.
Develop, write, implement, evaluate and revise, as necessary, individual's treatment plans in collaboration with the Treatment Team.
Provide individual and group supportive interventions and symptom management ensuring that immediate changes in the treatment plan are made, as necessary.
Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
Educate and support individual's families and advocate for individual's rights and preferences.
