National Child and Family Services (NCFS) is seeking a Wraparound Facilitator to join our Children Service's Division in our Harahan, LA location.

POSITION DETAILS
Position summary or overview. Responsibilities include but are not limited to the following.
Serves as the primary contact person for the family and collaboratively supports the development of the Individualized Plan of Care
Initiates contact with the family within (3) calendar days of receipt of referral
Conducts face-to-face with the family within (7) business days of assignment to discuss:
Wraparound goals
Wraparound process
Role of the referring agency
Expectations of Family/Caregiver
Expectations of child
Development of the Child/Youth and Family Team (CFT)
Client/Family/Caregiver safety issues
Creation of a crisis plan
Stability of housing, childcare, and respite needs
Works with Independent Evaluator (Licensed Mental Health Professional) to ensure that the comprehensive assessment is completed and the results are incorporated into the CFT process
Creates, convenes, coordinates and facilitates a Child and Family Team for each enrolled child/youth and family
Formalizes, monitors and modifies the Plan of Care developed by the CFT
Assures that families have a choice, sustained access to, and ownership of their Plan of Care and associated resources
In partnership with the family, develops an individualized Plan of Care that adheres to National Wraparound Initiative standards and treatment planning requirements consistent with 42 CFR 438.208(c)(3)
Submits the Plan of Care to the Statewide Management Organization (SMO) for review prior to the end of the initial 30-day authorization period
Works directly with the child/youth and family to implement elements of the Plan of Care
Convenes, coordinates and communicates with the identified collateral contacts to implement the Plan of Care
QUALIFICATIONS
Bachelor's degree in Human Services field, such as Psychology, Counseling, or Social Work
Note: Any applicant with a degree in Social Work must be in possession of active license/registration as required by the Louisiana State Board of Social Work Examiners
Minimum of two (2) years of full-time experience working in relevant family, children/youth or community service capacity
Must pass a Louisiana criminal history background check and motor vehicle screens
Must have access to a reliable, insured motor vehicle, with current, valid Driver's License
Must possess and understanding of child and adolescent development
Applicant must possess the appropriate credentialing/license/registration from professional boards that are necessary to perform assigned duties. All applicants with a degree in Social Work are expected to comply with Louisiana Social Work Practice Act, Title 37: §2709 - No individual shall practice social work in the state unless the individual holds a current, valid license, certificate, or registration issued by the board in accordance with this Chapter.
Rate of pay: $21.15/ hour
