The duties of the Community Support Specialist include contributing to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provides clinical assessment, counseling, and psychotherapeutic services to individuals, families, and groups. Conducts evaluations, including mental status examinations and psychosocial histories, to diagnose and develop individualized treatment plans using accepted diagnostic classifications (e.g., DSM). Delivers ongoing clinical social work and/or psychotherapeutic counseling to address mental health symptoms, psychosocial stressors, and relationship challenges. Engages in clinical consultation through case discussions, treatment planning, and client-centered advocacy, collaborating with relevant individuals and entities to support therapeutic goals. The Children's Intervention Specialist intervenes in crisis situations through mobile outreach response. Interviews clients, performs assessment and crisis interventions, creates individual crisis plans and provides case management by making linkage and referrals. Reaches out to post intervention clients to perform follow-up. Responds to crisis outreach immediately and is on-site of the crisis within one hour of receiving crisis outreach call.
