Job Description

Merakey is seeking a PQI Coordinator to join our Intellectual and Developmental Disabilities Services (IDD) team in Philadelphia Pennsylvania.


The PQI Coordinator is responsible for Incident management and Quality Initiative activities for the IDD Division, Philadelphia waiver services and programs as assigned.


Responsibilities include but may not be limited to:

  • Abides by established policies and procedures of the organization, County and State
  • Comprehensive knowledge of state regulations and incident management requirements of the state
  • Maintains valid PA certified Investigator certification
  • Completes investigations as assigned
  • Oversight of incident management database; designated point person for assigned caseload, data entry, coordinating the completion of EIM reports, adhering to established timeframes
  • Coordinate, maintain and analyze incident data using centralized system
  • Tracks trends and patterns to increase incident awareness and decrease occurrences
  • Monitors the quality and completion of reports and investigations
  • Monitor incident hotline and ensure completion of internal and external notification
  • Participates in PQI meetings and quality assurances activities as appropriate
  • Aids I/DD programs in quality assurances activities, trainings and/or initiatives as assigned
  • Supports program during surveys/inspections as needed
  • Participates and presents in Investigation review meetings
  • Participates in Certified Investigator Peer Review process
  • Remains flexible in meeting the needs of individuals, the respective programs and organization at large
  • May perform other related duties as assigned
  • Computer skills (Microsoft Word, Excel, Outlook)
  • Strong Reading/Comprehensive Skills
  • Strong Writing skills
  • Ability to work as a team
  • Strong Organizational, and Analytical skills.

Schedule: M-F 8:00 AM - 4:00 PM; rotating weekends
