Job DescriptionAt Coram/CVS specialty infusion services, we have an extremely rewarding opportunity for you to apply your exceptional pharmacy skills in a home infusion pharmacy. As a Compounding Home Infusion Pharmacist, you will work with specialized and complex therapies utilizing cutting edge technology. Use your clinical skills and infusion / TPN knowledge to provide specialty infusion services administered to patients in their homes. The Albany branch is seeking a Home Infusion Pharmacist available M-F, and to participate in the on call rotation. This is a full time position. As a CVS Health colleague, you'll receive a competitive benefits package that connects you with affordable, flexible and personalized programs for managing your overall well-being.Key responsibilities:- Accurately compounds and checks compounded sterile products.- Directly supervises pharmacy support personnel in the appropriate compounding, packaging and labeling of compounded sterile products.- Obtains, interprets and evaluates all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness- Initiates, reviews and updates all required documentation.- Accurately maintains all legally required records.- Communicates clinical and technical information and monitoring to other members of healthcare team and patient when appropriate.- Participates in patient evaluation for acceptance, product selection, device selection and supply management.- Performs accurate computer order-entry, labeling and dispensing in accordance with policies and procedures and state board of pharmacy regulations.- Follows established procedures for drug and supply procurement.- Complies with FDA mandates and self-imposed manufacturer restrictions on particular drugs, as applicable.- Maintains appropriate inventory levels and ensures use of preferred products.- Maintains procedures for appropriate security of all legend items, including controlled substances.- Participates in on-call responsibilities.- Participates in Performance Improvement activities, educational programs and departmental/branch meetings.- Participates in the branch assessment process as requested by pharmacy manager or designee.- Communicates appropriately with all members of the healthcare team.- Assists in training and orientation of new employees as requested by pharmacy manager or designee.- Complies with the Company's policy on the allowable clean room garb and dress code requirements.Learn more about Coram: https://www.youtube.com/user/CoramHealthcareRequired QualificationsActive NY pharmacist license in good standingPreferred QualificationsOne year related experience in an IV alternate site environment or equivalent is preferred.EducationB.S. or doctorate of pharmacy degree is required.Business OverviewIt's a new day in health care.
