MSHO/MSC+ Medica Care Coordinator

Position Purpose: The role of a Care Coordinator is to provide ongoing assessment, coordinate services and referrals, ensure access, provide advocacy, and help each member make informed choices as they navigate complex social service and health care delivery systems.

In this position you will:

  1. Work in partnership with the clients to develop an individualized comprehensive plan of care that will assist each client in addressing medical, social, behavioral, educational, and other service needs. Services and supports will include conducting a LTCC (Long-Term Care Consultation) or HRA (Health Risk Assessment), assess for the Elderly Waiver and/or PCA services, referrals and coordination with primary and dental care, and maintaining benefits.
  2. Be the members primary contact for accessing all benefits under MHSO/MSC+. As a representative of Medica, the Care coordinator manages benefits provided by state plan Home Care Services, Medicare, CADI or Elderly Waiver services (if applicable). The Care Coordinator is in the unique position of assisting the member across all settings of care, transitions, and stages of the aging process.
  3. Assists individuals in managing their transitions of care.
  4. Advocate on behalf of each individual to assure the protection of rights and privacy. Assure that individuals understand the complaint and grievance procedures.
  5. Provide collaboration and education to significant others and family members who are interested in assisting the individual in their health plan and recovery process.

Minimum Education and Experience:

MSHO/MSC+ Care Coordination must be provided by an individual that is a Registered Nurse, a Licensed Social Worker, Physician Assistant, Nurse Practitioner or Physician.
