Description: The position is dedicated to work with the health plan's MLTSS members, state representatives, district of community affairs representatives and care management team to facilitate the provision of housing. The housing specialist must live within the state of new jersey and will be responsible in identifying, securing and maintaining community-based housing for MLTSS members. The housing specialist will assist in developing and implementing Aetna's housing strategy. This person will act as a housing expert, provide housing resources, education, and assistance to care managers and members regarding housing and related issues for the MLTSS population. The housing will assist MLTSS members in applying for a voucher through open waitlists and have knowledge of subsidized and low-income housing. The housing specialist will be responsible for housing related work including nursing facility to community transition, monthly/quarterly reporting, and will act as a liaison with DMAHS staff. The person assign to this position must have at least two  years experience in health care working with low-income populations, and have demonstrated cultural sensitivity.
Fundamental Components: Investigate and resolve housing access identified by the health plan staff, State staff, providers, advocate organizations, and members. The housing specialist will collaborate with the MLTSS member advocates, community agencies, and State agencies. Recommend policy and procedural changes to the health plan's management including those needed to ensure/improve member access to housing, care, and quality of care; changes can be recommended for both internal administrative policies and provider requirements. Function as a primary housing contact for member advocacy groups; work with these groups to identify and correct member access barriers. Participate in local community organizations to acquire knowledge and insight regarding the special health care needs of members. Analyze systems functions through meetings with staff Organize and provide training and educational materials for the health plan's staff and providers to enhance their understanding of the values and practices of all cultures with which the health plan interact. Maintains availability/willingness to travel to various locations as frequently as necessary to meet business needs throughout the state of NJ. Maintains availability and willingness to work the schedule necessary to meet business needs. Works in a manner that is not disruptive to peers, supervisors and/or subordinates. Effective interpersonal skills and ability to act as a team player. Statistical analysis skills helpful. Ability to pay attention to detail, work in a fast paced/evolving environment. Capable of meeting deadlines on a consistent basis. Effective organizational skills and ability to manage multiple tasks. Proficiency with word processing, spreadsheet applications, database management and graphics experience. Microsoft Office Suite preferred Ability to successfully interact with members, medical professionals and health plan and government representatives. Effective telephone, oral and written communication skills. Bilingual preferred. Cultural Competency training experience preferred. Project management of special events experience preferred. Additional Job Information: Candidates must have knowledge of low income housing, section 8 vouchers, HUD, relocation assistance, and subsidized housing. Real estate or social work certification, preferred but not necessary with the above experience.
