Community Health Workers (CHWs) are frontline public health workers who are trusted members of and/or have an unusually close understanding of the community they serve. This trusting relationship enables CHWs to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
REPORTS TO: MANAGER, CARE COORDINATION

POSITION LOCATION: CHICAGO, IL
Educates clients on how to use the health care and social service systems.
Gathers information for medical providers and insurers.
Informs medical and social service providers about community needs.
Provides individual support and informal counseling.
Delivers Culturally Appropriate Health Education.
Assesses health risk factors and barriers to accessing care.
Develops a Personal Health Plan based on the client's vision for personal health and wellness.
Provides education and support on Lifestyle Modifications (healthy diet, importance of exercise).
Teaches concepts of health promotion and disease prevention.
Helps to manage chronic illness.
Provides assistance in navigating state healthcare systems.
Provides resources, referrals and linkages to health related services in the community.
Builds partnerships with individuals, families, youth, and key stakeholders to promote recovery, resiliency and wellness.
Establishes mechanisms to increase the member and family voice in areas of leadership, service delivery and related committees and boards.
Communicates and collaborates with members and families to identify concerns and remove barriers that affect service delivery or member satisfaction.
Promotes the development and use of member and family support programs.
Advocate for service environments that are supportive, welcoming and recovery oriented by implementing Trauma Informed Care service delivery approaches and other initiatives.
Collaborates with state or local agencies.
High School or GED is required
2 years of experience in health or social services is preferred
Ability to effectively present information and respond to questions from families, members, and providers
Ability to work independently
Demonstrated interpersonal/verbal communication skills
Licenses and Certifications: A license in one of the following is required:
Valid driver's license (or readily available mass transit) is required
Microsoft Word is required
