Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. We follow a unit based model of care, with a nurse manager overseeing the overall functioning of each residential unit. . Each patient has a primary nurse who coordinates all his/her care needs. Growing Abilities Duties of this position include: Demonstrates effective assessment skills by collecting and analyzing data, interviewing, observing, and examining patients, and through review of the patient's medical record. Formulates a comprehensive and unique plan of care for each patient; monitors patient outcomes to treatments and revises the plan as needed. Demonstrates critical thinking skills to determine appropriate nursing interventions and/or the need for collaboration with the physician or other clinicians. Carries out all nursing/physician orders and documents nursing care and patient responses in accordance with hospital requirements. Works with and supervises the PCA staff / unlicensed assistive personnel's work, who do the direct patient care of ADL's, bathing assisting with eating, etc. Demonstrates proficiency in all fundamental nursing skills. Understands and carries out the roles and responsibilities of charge nurse when assigned. Applies CDC Infection Control Guidelines for Standard Precautions and Transmission-Based Precautions and monitors infection control practices within the work environment. Participates as a member of the interdisciplinary team by giving and receiving pertinent information to colleagues. Maintains on-going and timely communication with families/guardians on all patient-related issues. Advocates effectively to ensure communication of patient information when transitioning patients between healthcare professionals, both internally with the MCMD and externally with outside providers. Demonstrates appropriate communication with patients with consideration of their age and functional levels. Communicates effectively with direct care providers regarding patient care needs. As a Registered Nurse in a special hospital, performance of additional responsibilities that are not outlined in this job description may be required in an emergency situation. Additional assistance as a result of staff absences or shortages is also required, including additional weekend and/or holiday time.
