The Supported Living program currently supports 17 individuals who maintain independent housing arrangements throughout the Blackstone Valley area. Individuals served receive between 1 and 30 hours per week of individualized staffing assistance tailored to his/her needs and preferences. Individuals have opportunities to develop meaningful connections, participate in or volunteer for community activities. The program emphasizes independent living skills, self-advocacy, and building and maintaining relationships with family, friends, and peers. Responsibilities Assist the Program Coordinator in the management of the budget and the daily program schedule; troubleshoot scheduling issues as they arise. Assume Program Coordinator responsibilities in the absence of the Coordinator. Assist in developing and implementing individualized service plans( ISPs). Assist individuals to link to, manage and coordinate all community resources. Facilitate the development of valued community roles and relationships. Assist individuals with completing daily routines. Ensure health and safety of individuals and the cleanliness of their apartments. Implement all prescribed medical and medication protocols. Assist individuals to access and/or provide all needed transportation. Participate in team meetings, supervisions, and other assigned meetings. Review and complete all required documentation. Communicate pertinent information to stakeholders. Provide crisis intervention as needed and participate in regularly scheduled on-call beeper rotation. Acquire and maintain all required certifications. Qualifications Bachelor’s degree in Social Services and supervisory experience preferred. Valid Driver’s License and High School Diploma/GED required.
